Appointments and Frequently Asked Questions:
Where is your office located?
5728 Major Blvd., Suite 700
Orlando, Florida 32819
Do you offer tele behavioral health or video counseling?
Yes. I utilize HIPAA compliant Televideo platforms. It is required to complete the initial intake form and sign the video consent statement form, before the first session. Please refer to these sections on this website.
How long are the sessions?
Each session is generally 50 - 60 minutes long.
Do you see military personnel and their families?
Yes. Military families are exclusively seen through Tricare/Humana.
How often should we meet?
It is recommended to meet once a week. The flow of counseling is more effective if this schedule is consistent.
What is an "EAP", and are you on these type of panels?
EAP is short for: Employee Assistance Program. This is a plan offered through your workplace that has a counseling
coverage component- simply check with your employer to see if a EAP is available. This service can offer anywhere from 3 - 12 counseling sessions free of charge for you and/or anyone who lives with you currently. Calling your provider will inform you how many sessions you're eligible for. I currently accept all major EAP plans. Employee Assistance Programs can be provided by a different company from your major insurance plan.
What if I want to use insurance?
I accept most major insurance for counseling and clinical services.
Please refer to the intake section to provide the basic insurance information. We will be happy to verify and inform you
of benefits before our first session. Please include the policy holder's: name, date of birth, ID number, holders mailing address,
and a toll free number of the insurance company. Your information is submitted confidentiality through the secure
portal provided.
You may be responsible to meet a deductible before the insurance coverage goes into effect. If this is the case, you can remit what the carrier would pay for each session. Co-insurance payments may also be owed after each session.
What if I want to pay you privately?
Sometimes, clients will choose to self pay for increased confidentially. If you do not want any billing involved, or if you do not have a plan that covers counseling services: I do offer a sliding scale fee schedule. This is dependent upon what your household income is per annum.
What methods of payment do you accept?
Cash, checks, HSAs or major credit cards are all accepted.
When is payment due?
All deductible, co-insurance, or session fees are due at the beginning of the meeting.
